INTAKE FORMs & POLICIES


For us to give you the best service  it is extremely important that you are honest about any HEALTH ISSUES that we should be aware of.  For example we CANNOT perform spa services if you are in the first trimester of pregnancy, or have a cold sore, ring worm or rashes of any kind. Other things we may need a doctor’s note for or just adjust our treatment. Some medical or mental health conditions may be out of our expertise or out of our scope of practice to treat.  By having this information ahead of time we are able to give the best treatment possible or refer out when appropriate. 


 If you do not disclose but come in any way we can not see you and will have to charge you a cancellation fee.  This fee may be as low as $59 and as high as the cost of the service.



We take COVID-19 Precautions very seriously. WE CAN NOT SEE YOU If you are icky sneezy, coughy, weezy, if you've been exposed or think you may have been exposed. If within 48 hours of your appointment to avoid the cancellation charge you may gift your appointment slot to another person.

*** You must fill out your own intake. Someone else may type your answers for you but you must answer the questions yourself. 

Intake forms are required by the PA massage licensing board and in addition, PA law requires Electronic Health records.

Serenity Wellness Center is proud to provide secure electronic client intake forms. 


Once you complete the Intake form, call us at 610-565-6627 to review the information. We are happy to schedule you once this is completed. Please do not skip any questions as you will need to start all over again. This is the basic information needed to schedule your service. You will have a longer MEDICAL intake to fill out your day of service.   

If you have a voucher/gift card please have this information with you to fill into the intake form. We will also need a Credit card or Debit card to schedule you.

By completing this intake and scheduling with Serenity Wellness LLC you agree that you have read and agree to abide by the following terms and conditions of all services at Serenity Wellness LLC DBA Serenity Wellness Center, Serenity Massage & Spa, Serenity Counseling & Wellness: If any part of this waiver shall be held invalid, the balance shall not withstanding continue in full legal force and effect. I understand by scheduling that I agree to these policies for this and all future services at Serenity Wellness LLC.

Cancellation

We require a 48-hour notice for all cancellations and rescheduling. Please call 610-565-6627 to cancel/reschedule all appointments. Please leave a message if after hours or if we are busy helping other clients.


Cancellations less than 48 hours before your appointment or No shows will be charged 100% of the full-service charge as we do not have time to schedule another client in your place. You may gift your appointment slot to another person, just have them fill out the intake form above, tell them about the gratuity and where to park, and give us a quick call to tell us who will be taking your place. (Groupon clients are charged a $59 cancellation fee You will however retain your Groupon certificate to reschedule.)

If less than 48 hours before your appointment you may gift your appointment slot to another person to avoid the cancellation charge.  We can not have additional people in our waiting room so your friends/family etc will have to wait outside during your appointment.

If for any reason we cannot charge your card, gift certificate clients will forfeit their gift certificate.

We recommend you have a sitter in place in case your child is sick as we cannot reschedule due to the child home from school. We also cannot reschedule when you are called into work. We are happy to reschedule when you give us 48 hours cancellation notice.

On rare occasions we may wave the fee, however, this is on a case by case basis. We must be strict with our policy as we have reserved this time just for you and cannot fill the slot without proper notice. Our time is valuable please respect us by giving time to fill your appointment slot. 

Health Contraindications

Other things we may need a doctor’s note for or just adjust our treatment. If you do not disclose but come in any way we cannot see you and will have to charge you a $59 cancellation fee. We will also charge that cancellation fee if you show up with a sunburn.

Weather Emergencies

In case of a snow emergency, tornados, power outages, etc., the facility will be closed for our safety and yours. No fee will be charged in dangerous conditions. We post our closings on our Facebook site, Serenity Wellness Center. 


Gratuity

Gratuity is not included in the price of the service. Nor is it included in your Groupon certificate. We ask that you bring cash for gratuity as we cannot accept credit or debit cards for gratuity. Standard gratuity is 15-25% or more of the pre-discounted service price. Please be generous with your therapist when earned. Please tell us if service is lacking in any way, as we rely on your feedback so that we may constantly improve services. We are aware that many receive the service as a gift and have no idea what the original price was so for your convenience, our receptionist will give you standard Gratuity suggestions based on the original pre-discounted price. 

Lateness

If you are late, you may not be able to receive my full appointment time. However, you will still be charged for the scheduled service in full. 

Testimonials

If you chose to give a written or video testimonial or review, we reserve the right to use that testimonial /review in advertisement and or marketing.

Refunds

No refunds on gift certificate purchases, product purchases, or appointments scheduled and not canceled within standard 48 hours cancellation notice.

Children and pets

Registered Service dogs are always welcome but we cannot have you bring your other non-service pets. Unfortunately, we cannot accommodate small children, infants, toddlers, etc. For their safety and the comfort of other guests receiving treatments, please arrange for a sitter at home.

Parking & Location

We are located at 349 W. Baltimore Ave in Media PA on the corner of Baltimore Ave and Lemon Street, diagonal from Hill's Seafood. We are in the little town of Media. If traveling south, we are at the end of Media on the right-hand side. We are the last building in the block with the Wine and Spirits store. If you go down a very steep curved hill on the way to the old Granite Run mall you have gone too far. 

If traveling North, from Granite Run, you will pass the Sterling Pig on the left and we are at the crest of the hill, on the left-hand side.
